河口から約64㎞まで来ると、流域でも有数の規模の都市であるストックトンの西縁部を通る。ここから河口までは航行維持事業(Stockton Deep Water Ship Channel)の一部として浚渫が行われている。満潮汀線より下流では三角州が作り出した多くの島の間を通り、カラベラス川(英語版)、モケルミ川(英語版)が合流する。サクラメント川との合流部手前、アンティオックでは、川幅は約1,500mまで広がる。川は最終的にサスーン湾へと注ぐ。サクラメント川と合流した後はカルキネス海峡を抜け、サンフランシスコ湾から太平洋へと注ぐ[1]。
