איבוד חלבונים אנטרופטי (באנגלית: Protein losing enteropathy בקיצור PLE) היא תסמונת אובדן יתר של חלבוני דם דרך מערכת העיכול. הדבר עלול להיגרם על ידי מחלות בסיסיות רבות ושונות הפוגעות בקרום הרירי של מערכת העיכול או על ידי מחלות הגורמות לחסימה של ניקוז מערכת הלימפה שלה.[1]

סימנים ותסמינים

הסימנים/תסמינים של איבוד חלבונים אנטרופי תואמים לשלשולים, חום ואי נוחות כללית בבטן.[2] גם נפיחות רגליים כתוצאה מבצקת היקפית יכולה להתרחש, עם זאת, אם ה-PLE קשור למחלה מערכתית כגון אי-ספיקת לב או פריקרדיטיס מכווץ (דלקת קרום הלב המכווץ הוא מצב רפואי המאופיין במעטפת מעובה, פיברוטית, המגבילה את יכולת הלב לתפקד כרגיל) אז הסימפטומים עשויים להיות של התפתחות המחלה הראשונית.[3]

מנגנון

פרוטאוגליקן

הפתופיזיולוגיה של איבוד חלבונים אנטרופטי היא תוצאה של אובדן חלבוני פלזמה, אשר נכנס למערכת העיכול (נהור).[3] PLE הוא סיבוך של הפרעה, בין אם זה חסימת לימפה או פגיעה ברירית.[5]

באיבוד חלבונים אנטרופטי בילדים ישנם מספר שינויים בתאי אפיתל הגורמים ל-PLE על ידי הגברת קצב זרימת החלבונים (סרום). הפרעות מולקולריות מולדות, או ניקוז לימפתי לא מתפקד עלולים לגרום לשינויים במטריצת האפיתל. פרוטאוגליקנים, שהם שרשראות גליקוזאמינוגליקן המחוברות לחלבון, גורמים ישירות ל-PLE, כמו גם מגבירים ציטוקינים דלקתיים. לילדים שיש להם פגמים מולדים של גליקוזילציה יש בדרך כלל איבוד חלבונים אנטרופטי.[6]

אבחון

האבחון של תסמונת איבוד חלבונים אנטרופטי נעשית על ידי אי הכללה של גורמים אחרים לאובדן חלבון. ניתן להשתמש באנדוסקופיה כדי לאתר את הסיבה לאיבוד החלבונים במעי. שיטות שונות כוללות הפרשת צואה של אלפא 1-אנטיטריפסין שהוא סמן לתסמונת איבוד חלבונים אנטרופטי, כמו גם סרולוגיות ויראליות שעשויות להיות שימושיות לאיתור PLE.[4]

טיפול

טיפול בתסמונת איבוד חלבונים אנטרופטי תלוי במצב הבסיסי, לפי Rychik, זה יכול להיות טיפול בהיפופרוטאינמיה או ברירית המעי.
