Meloksikam (po IUPACu 4-hidroksi-2-metil-N-(5-metil-2-tiazol)-2H-1,2-benzotiazen-3-karboksamid-1,1-dioksid) spada u grupu nesteroidnih protivupalnih lijekova.[1] Derivat je oksikama, sa snažnim analgetičkim, antiinflamatornim, antipiretičkim djelovanje i antiagregacijskim djelovanjem.

Djelovanje

Meloksikam je derivat oksikama, sa snažnim analgetičkim, antiinflamatornim, antipiretičkim djelovanje i antiagregacijskim djelovanjem. Inhibira enzim ciklooksigenazu koji je uključen u sintezu prostaglandina, troboksana i prostaciklina, koji su glavni medijatori upale i bola.[2]

Indikacije

Meloksikam se upotrebljava u simptomatskom tretmanu reumatoidnog artritisa i osteoartritisa (artroze, degeneratisvne promjene zglobova).

Kontraindikacije

  • Preosjetljivost na sastojke lijeka ili druge nestereoidne protivupalne lijekove
  • Oboljenja digestivnog trakta praćena hiperaciditetom i ulkusnim projenama
  • Oštećenje koštane srži, teška oštećenja jetre i bubrega
  • Hronična astama, nazalni polipi
  • Djeca i adolescenti mlađi od 15 godina

Neželjeni efekti

Neki od neželjenih efekata su:

  • mučnina, bol i nelagoda u želucu
  • konstipacija, dijareja, rjeđe nadutost, povraćanje, aftozni stomatitis, promjene apetita, suhoća usta
  • peptički ulkus sa krvarenjem
  • glavobolja, vrtoglavica, depresija, pospanost, fotosenzitivnost
  • rijetko hematurija, alergijske reakcije, hipertenzija, trombocitopenija, autoimuna hemolitička anemija

Posebna upozorenja

Potreban je oprez kod pacijenata sa peptičkim i duodenalnim ulkusom, srčanom insuficijencijom, hipertenzijom ili drugim stanjima koja dovode do retencije tečnosti u organizmu. Oprez je potreban i u pacijenata s astmom, bronhospazmom, kardiovaskularnim oboljenjima, poremećajima koagulacija|koagulacije i funkcije trombocita, oslabljenom funkcijom jetre i bubrega.

Meloksikam je dostupan u obliku tableta, te kao injekcije za intramuskularnu primjenu.
