Vyrážka u človeka s porfýriou

Porfýria je autozomálne dedičné (vrodená erytropoetická porfýria sa dedí recesívne) metabolické ochorenie, ktorého následkom je porucha tvorby hému. Za klinické symptómy sú zodpovedné metabolity, ktoré sú pred defektným enzýmom alebo nedostatok tých, ktoré sú za defektným enzýmom (produktom zmutovaného génu) metabolického reťazca syntézy hému.

Príčina vzniku

Na syntéze hému sa podielajú tieto enzýmy :

  1. ALA syntáza,
  2. ALA dehydratáza,
  3. uroporfyrinogén-I syntáza,
  4. uroporfyrinogén-III kosyntáza,
  5. uroporfyrinogéndekarboxyláza,
  6. koproporfyrinogénoxidáza,
  7. protoporfyrinogénoxidáza,
  8. ferochelatáza.

V prípade poruchy na 3. enzýme pôsobia delta-aminolevulát (ALA) a profobilinogén (PBG), ktoré sa hromadia v telesných tekutinách, toxicky na brušné nervy a centrálny nervový systém, čo vedie k abdominálnym bolestiam a neuropsychickým problémom. Enzýmové bloky v neskorších stupňoch metabolizmu majú za následok hromadenie metabolitov v tkanivách. Pri hromadení v koži následnou oxidáciou vznikajú molekuly citlivé na svetlo. Fotoexcitáciou týchto molekúl vznikajú vysoko aktívne molekuly atakujúce kyslík za vzniku kyslíkových radikálov, ktoré sú zodpovedné za kožné prejavy fotosenzitivity (svetlocitlivosť je jeden z príznakov).

Druhy

Porfýrie sú klasifikované podľa najviac postihnutých orgánov. Známy je zvýšený lokálny výskyt porfýrie v belošskej populácii Juhoafrickej republiky, ktorý je spôsobený genetickým driftom. Predpokladá sa, že zmenený vzhľad neliečených a fotosenzitivita viedli v minulosti k vzniku historiek o upíroch.

Liečba

Terapia porfýre zatiaľ nie je. Indikovaná je len symptomatická liečba, ktorá zmierňuje príznaky. Napríklad podanie betakaroténu, ktorý vďaka svojmu antioxidačnému účinku zmierňuje kožné prejavy. Používané sú tiež slnečné filtre. Príznaky porfýrie zhoršujú mnohé látky. Mechanizmus tohto účinku spočíva v tom, že sa na ich odbúravaní týchto látok podieľa cytochróm P450, ktorého súčasťou je protoporfyrínový kruh. Používanie cytochrómu P450 indukuje syntézu porfyrínov a teda ukladanie aktívnych metabolických prekurzorov. Medzi látky, ktoré toto spôsobujú, patria napríklad barbituráty, anestetiká a alkohol. Porfýrie sú adeptom génovej terapie.
