Extraliga hádzanárov v sezóne 2008/09 bol v poradí 16. ročník extraligy hádzanárov, spravovanej Slovenským zväzom hádzanej. Účinkovalo v nej 8 družstiev.[1][2] Titul tretíkrát v rade získalo družstvo HT Tatran Prešov, ktoré vo finálovej sérii zvíťazilo nad IS Košice.[3]

Prešov hral opäť popri slovenskej extralige aj základnú časť českej extraligy, ktorú opäť vyhral.[4]

Systém súťaže

Systém súťaže sa oproti predchádzajúcim sezónam zmenil, zjednodušil sa. Nehrala sa ani nadstavbová časť, ani prelínacia súťaž. Najprv sa hrala štvorkolová základná časť, ktorá pozostávala z 28 kôl. Každý s každým sa stretol dvakrát doma a vonku. Po základnej časti 4 najlepšie družstvá postúpili do semifinále play-off. Všetky duely play-off sa hrali na 3 víťazné zápasy. Posledné družstvo po základnej časti zostúpilo v nasledujúcej sezóne do 1. ligy, kde ho nahradilo víťazné družstvo nižšej súťaže.[2]
