צילום רחם (באנגלית: Hysterosalpingography; ידוע גם כ-HSG וכ-Uterosalpingography[1]) הוא הליך רדיולוגי, שנועד לחקור את הצורה של חלל הרחם ואת הצורה ואת העבירות של צינורות החצוצרות. בהליך זה, מזריקים חומר ניגוד לתוך תעלת צוואר הרחם וגם פלואורוסקופיה עם הגדלת תמונה. תוצאה נורמלית מראה את מילוי חלל הרחם ומילוי דו צדדי של החצוצרות עם חומר ההזרקה. קרע בצינורות נצפה כאשר יש דליפה של החומר לתוך חלל הצפק. לצילום תפקיד חיוני בטיפולי פוריות, במיוחד במקרה של חסימת צינור החצוצרות.

נוהל

ההליך כולל צילום רנטגן והוא צריך להיעשות בשלב הפוליקולרי של המחזור החודשי[2]. זה התווית היריון. ההליך יעיל לאבחון מומים ברחם, תסמונת אשרמן, חסימת חצוצרות ולמחלה דלקתית של האגן[3]. כמו כן, צילום רחם נעשה בשימוש רחב על נשים עקרות[4]. כמו כן, טוענים כי הסיכוי להיריון עולה לאחר ביצוע הצילום[5][6][4]. באמצעות קטטר, התערבות רדיולוג או רדיוגרף מאומן במיוחד יכולים לפתוח צינורות שנסגרו באופן מובהק. הליך זה נעשה בדרך כלל עם חומר ניגוד רדיוגרפי (צבע) אשר מוזרק לתוך חלל הרחם דרך הנרתיק וצוואר הרחם. אם צינורות החצוצרות לא חסומות, חומר הניגוד ימלא את הצינורות ויישפך לתוך חלל הבטן. ניתן לקבוע האם צינורות החצוצרות פתוחים או חסומים והאם החסימה ממוקמת בצומת הצינור והרחם (הפרוקסימלי) או אם הוא נמצא בקצה החצוצרה (דיסטלית). צילום הרחם יכול להיות מכאיב, ולכן משככי כאבים עשויים להינתן לפני או לאחר ההליך כדי להפחית את הכאב. רופאים רבים גם ממליצים על נטילת אנטיביוטיקה לפני ההליך כדי להפחית את הסיכון של זיהום.

תועלת טיפולית

צילום רחם נחשב הליך אבחוני, אך יכולים להיות לו גם יתרונות טיפוליים בטיפולי פוריות. חומרי ניגוד על בסיס שמן משפרים את שיעורי ההיריון בכ-10% לעומת חומר ניגוד על בסיס מים[4]. מטא-אנליזה גילתה 3.6 סיכויים גדולים יותר (OR=3.6) של היריון עם חומר ניגוד על בסיס שמן לעומת אי ביצוע צילום רחם[6]. השפעה זו נחשבת בשל שטיפת הצינורות עם חומר הניגוד על בסיס שמן ולא הליך ההדמיה עצמה.

התוויות נגד

נשים הרות אינן יכולות לעבור הליך של צילום רחם. ההליך כולל סיבוכים הכוללים: זיהום, תגובות אלרגיות לחומרים בהם משתמשים לצורך ההליך ואם משתמשים בחומר המבוסס על שמן, יכול להופיע תסחיף ריאתי. בנוסף, אוויר יכול לחדור בטעות לתוך חלל הרחם על ידי מבצע ההליך, לכן יש להגביל את הבחינה בשל סיבוכים שקשורים לפעולה. אפשר להתגבר על בעיה של חדירת בועות אוויר על ידי Tenzer Tilt, אשר ידגים תנועה של בועות אוויר לחלק העליון של חלל הרחם.

היסטוריה

צילום הרחם הראשון בוצע בשנת 1914 על ידי שימוש בחומר ניגוד קולגרול. בשנת 1924, סיקרד ופורסטייה הציגו את חומר הניגוד ליפידול, אשר נותר פופולרי במשך עשורים רבים[7]. מאוחר יותר, חומר ניגוד מסיס במים היה מועדף בדרך כלל משום שהוא מונע סיבוך אפשרי של תסחיף שמן.

מעקב

אם צילום הרחם מצביע על ממצאים נוספים, ניתן לבצע לפרוסקופיה בשילוב עם היסטרוסקופיה על מנת להמחיש את השטח בשלושה ממדים, עם פוטנציאל לפתור בעיות קלות בתוך אותו הליך.
