עצם ססמואידית

שתי עצמות ססמואידיות בקצה המרוחק של עצם המסרק הראשונה של כף הרגל

עצם ססמואידית (TA: Ossa sesamoideum ידועה גם כ-"שומשומידית") היא עצם השקועה בתוך גיד, ונוצרת ממנו בתהליך של הסתיידות[1]. שמה הלטיני של העצם נגזר מהמילה sesamum שמשמעותה גרגר שומשום, וזאת לאור גודלן וצורתן של מרבית העצמות הססמואידיות. נוסף על העצמות הססמואידיות הנמצאות אצל מרבית האוכלוסייה ואלו המוגדרות כוריאנט אנטומי (אנ') תקין, עצמות אלו עשויות גם להתפתח כתוצאה משחיקה או מלחץ במפרק[2].

תפקיד העצמות הללו הוא לשמש כמעין גלגלת באזורים מפרקיים, ובכך הן משפרות את יעילותם של השריר והגיד בהפעלת כוח. לדוגמה, עצם הפיקה, העצם הססמואידית הגדולה ביותר בגוף, משמשת כגלגלת בעת הפעלתו של השריר הארבע-ראשי שתכליתו ליישר את הברך. הפיקה מחוברת בחלקה העליון לגיד הארבע ראשי ובחלקה התחתון אל רצועת הפיקה אשר מחוברת אל עצם השוקה. כאשר השריר הארבע ראשי מתכווץ, עצם הפיקה פועלת כמנוף על נקודת משען בקצה התחתון של עצם הירך, וממירה את כיוון כוחו של השריר.[1]

תהליך ההסתיידות של העצמות הססמואידיות מהווה אינדיקטור לשלב ההתבגרות והגדילה של הפרט, וקורה בנערות לפני נערים.

בבני אדם

באופן תקין, אצל מרבית האוכלוסייה העצמות הססמואידיות נמצאות במפרקים שונים, כגון:

וריאנטים אנטומיים נפוצים

קיימות עצמות ססמואידיות שאינן נמצאות אצל רוב האוכלוסייה, אך הן אינן פתולוגיות ומדובר בווריאנט אנטומי תקין:

  • בכפות הידיים והרגליים, ישנם וריאנטים אנטומיים רבים כמפורט בתמונות המצורפות. אחד הוריאנטים הנפוצים הוא איחוד של שתי העצמות הססמואידיות הנמצאות במפרק הבוהן הגדולה לכדי עצם אחת, או מאידך פיצול של אחת מהעצמות (או שתיהן) לשני חלקים שונים.[4][5]
  • פבלה (Fabella) עצם ססמואידית המתפתחת בתוך גיד של הראש הלטרלי של שריר הסובך, הממוקם בצידה החיצוני של גומץ הברך. בבני אדם העצם מצויה אצל 10-50%[6] מהאוכלוסייה באופן טבעי, והיא נפוצה יותר אצל יונקים אחרים. עצם זו עלולה לגרום לכאבי ברכיים[7], אך כאמור היא איננה נחשבת כממצא גורם מחלה.[8]
  • ציאמלה (Cyamella) עצם ססמואידית קטנה הנמצאת בתוך גיד שריר הפופליטאוס (popliteus muscle) הממוקם בחלקו האחורי של מפרק הברך. העצם נדירה בבני אדם אך נפוצה יותר בפרימטים אחרים ובבעלי חיים אחרים.[9]

חשיבות קלינית

בעיה קלינית נפוצה המערבת את העצמות הססמואידיות היא ססמואידיטיס (אנ') - דלקת של העצמות הססמואידיות בכף הרגל. דלקת זו מאופיינת בכאב בזמן דריכה על הרגל. הדלקת נפוצה יותר בקרב ספורטאים אשר עיקר פעילותם מרוכזת בכפות הרגליים, כדוגמת אצנים ורקדנים.[1]

כמו כן, ייתכן כי יהיה שבר באחת העצמות הססמואידיות. שבר כזה הוא בדרך כלל קשה לאבחון משום שהוא עלול להראות כשתי עצמות נפרדות, במקום עצם אחת שבורה.[10]

בנוסף לכך, בגלל אספקת דם נמוכה לעצמות הססמואידיות, עלול להיגרם נמק בעצם שיוביל למות הרקמה.[11]
