עומס נגיפי

עומס נגיפי (בלועזית: עומס ויראלי) הוא ביטוי מספרי לכמות הנגיף בנפח נתון של נוזלים;

עומס נגיפי נמדד לעיתים קרובות כחלקיקים נגיפיים, או חלקיקים מדבקים לכל מ"ל, תלוי בסוג הניתוח. עומס נגיפי גבוה יותר מתואם לרוב לחומרתו של זיהום נגיפי פעיל. ניתן לחשב את כמות הנגיף למ"ל על ידי אומדן הכמות החיה של הנגיף בנוזל המעורב. לדוגמה, ניתן לתת אותו בעותקי RNA למיליליטר פלזמה בדם.

מעקב אחר עומס נגיפי משמש לניטור הטיפול במהלך זיהומים כרוניים נגיפיים, ובחולים חסרי חיסון כגון אלו המחלימים ממח העצם או השתלת איברים מוצקים. נכון לעכשיו, בדיקות שגרתיות זמינות לנגיף HIV-1, ציטומגלווירוס, נגיף הפטיטיס B, וירוס הפטיטיס C. מעקב אחר עומס נגיפי ב-HIV הוא בעל עניין מיוחד בטיפול באנשים עם HIV, מכיוון שזה נדון ללא הרף בהקשר של ניהול HIV / איידס.

טכנולוגיות לבדיקת עומס נגיפי

מחקר שנערך בשנת 2010 על ידי Puren et al[2] סיווג בדיקות עומס נגיפי לשלושה סוגים:

  1. בדיקות מבוססות הגברה של חומצות גרעין (NAT או NAAT) הזמינות מסחרית בארצות הברית באישור מינהל המזון והתרופות (FDA), או בשוק באזור הכלכלי האירופי (EEA) עם הסימון CE
  2. "NAT-brew" או NAT-in-house

דגימות פלזמה

פלזמה של EDTA היא מקור טוב ל־RNA נגיפי ללא תאים לבדיקת עומס נגיפי מבוסס RNA. תמורה של דגימה אוספת, אחסון בטיחות ביולוגי אמצעים חיונית. הפקת RNA מפלזמה דורשת ציוד מיוחד, מגיבים והכשרה, ומציבה אותו מחוץ להישג ידם של מעבדות בינוניות עד קטנות עם משאבים מוגבלים. יש צורך במדגם גדול (יותר ממיליליטר של פלזמה) לטווח ליניארי שפחות מ־50 למ"ל, הדורש נטילת דם ורידי. טווח ליניארי זה הוא הטוב ביותר לניטור הטיפול. אם מקובל טווח ליניארי גבוה יותר מאלף עותקים למ"ל, מקל אצבע יספק דגימה מספקת לאבחון של הידבקות ב־HIV בתקופת הינקות.[דרוש מקור]

אחסון

ניתן לאחסן פלזמה של EDTA בטמפרטורת החדר למשך 30 שעות, 14 יום בשעה 4°C ופרקי זמן ממושכים ב-70°C ללא ירידות משמעותיות באות העומס הנגיפי. ה-RNA בדגימות דם קטנות יותר, כמו כתמי פלזמה מיובשים (DPS) או כתמי דם יבשים (DBS) ממקלות אצבע, על פי הדיווחים יציב בטמפרטורת החדר לפרקי זמן הנעים בין 4 שבועות לשנה אחת. הנגיף מושבת בדגימות מיובשות, מה שמפחית את הסכנה מטיפול בדגימות. DBS ו-DPS הוערכו בהצלחה לבדיקת עומס נגיפי, אך הטווח הקווי שלהם הוא 3Log10 או 4Log10 עותקים למ"ל. בגלל חוסר רגישות זה, דגימות מיובשות שימושיות להקרנת HIV אך לא לקביעת עומס נגיפי.[דרוש מקור]

מדידה

עומס נגיפי מדווח בדרך כלל כהעתקים של HIV במיליליטר (מ"ל) של דם. שינויים בעומס נגיפי מדווחים בדרך כלל כשינוי יומן (בחזקות של 10). לדוגמה, עלייה פי שלושה בעומס נגיפי (3Log10) היא גידול של 103 או 1000 פעמים הרמה שדווחה בעבר, תוך ירידה מ־500,000 ל-500 עותקים תהיה ירידה (גם 3Log10).[דרוש מקור]

גורמים נוספים המשפיעים על עומס נגיפי

שיטות בדיקה שונות לעיתים קרובות נותנות תוצאות שונות עבור אותו מדגם מטופל. כדי להיות דומים יש להשתמש באותה שיטת בדיקה (הגברה של מטרה, הגברה ספציפית של בדיקה או הגברה של אות) בכל פעם שמפעילים דגימת חולה. באופן אידיאלי בדיקות חולים צריכות להתבצע באותה מעבדה רפואית, תוך שימוש באותה בדיקת עומס נגיפי ואבחון. זמן, עייפות ולחץ יכולים להשפיע גם על ערכי עומס נגיפי. חיסונים או זיהומים אחרונים עלולים להשפיע על בדיקת העומס הנגיפי. יש לדחות את הבדיקה בארבעה שבועות לפחות לאחר חיסון או זיהום.
