As far as I can tell, the use of the Cantelmo-Stuart name started with the children of Giacomo's brother Giuseppe, who married Diana Gaetani dell'Aquila d'Aragona. Diana was rather remotely related to James II of England through a common descent from Ferdinand I of Aragon, and slightly more closely related to Mary of Modena (and her son James Francis Edward Stuart) through a common descent from Ferdinand I of Naples. Giacomo's brother Restaino Cantelmo-Stuart presumably adopted the name on his marriage to his niece (Giuseppe and Diana's daughter) Beatrix, and its use continued with their descendants. Giacomo himself appears to have been unrelated to the Stuarts.
