Toksisitas ginjal

Toksisitas ginjal atau nefrotoksisitas adalah toksisitas pada ginjal . Toksisitas ginjal merupakan efek beracun dari beberapa zat, baik bahan kimia beracun dan obat - obatan, pada fungsi ginjal. Toksisitas pada ginjal memiliki berbagai bentuk, dan beberapa obat dapat memengaruhi fungsi ginjal dalam lebih dari satu cara. Nefrotoksin adalah zat-zat yang menunjukkan nefrotosisitas.

Nefrotoksisitas tidak boleh dikacaukan dengan fakta bahwa beberapa obat sebagian besar diekskresikan oleh ginjal dan perlu penyesuaian dosis pada seseorang yang memiliki penurunan fungsi ginjal (misalnya heparin, lithium).

Efek nefrotoksik dari sebagian besar obat lebih kentara pada pasien yang sudah menderita gagal ginjal.

Kardiovaskuler

Efek tubular langsung

Nefritis interstitial akut

Antibiotik β laktam, vankomisin, rifampisin, sulfonamid, siprofloksasin, NSAID, ranitidin, simetidin, furosemid, tiazid, fenitoin.

Nefritis interstitial kronis

Glomerulonefritis akut

Penyakit glomerular yang diinduksi obat tidak umum tetapi beberapa obat yang terlibat. Lesi glomerulus terjadi terutama melalui jalur yang diperantarai imun daripada melalui toksisitas obat langsung.

  • Timbal, merkuri dan garam kadmium [3]
  • Aristolochic acid, ditemukan pada beberapa tanaman dan dalam beberapa suplemen herbal yang berasal dari tanaman tersebut, telah terbukti memiliki efek nefrotoksik pada manusia.
  • Rhubarb mengandung beberapa nefrotoksin yang dapat menyebabkan peradangan ginjal pada beberapa orang.
  • Asam fumarat, atau juga dikenal dengan bahan tambahan makanan E297
  • Orellanine

Diagnosis

Nefrotoksisitas biasanya dipantau melalui tes darah sederhana. Penurunan bersihan kreatinin menunjukkan fungsi ginjal yang buruk. Level kreatinin normal adalah antara 80 - 120 μmol/L. Dalam radiologi intervensi, semua tingkat pembersihan kreatinin pasien diperiksa sebelum prosedur.

Kreatinin serum adalah ukuran lain dari fungsi ginjal, yang mungkin lebih bermanfaat secara klinis ketika berhadapan dengan pasien dengan penyakit ginjal dini.

Etimologi

Kata nefrotoksisitas (/ˌnɛfroʊtɒkˈsɪsɪti/) menggunakan bentuk menggabungkan dari nephro- + tox- + -icity, menghasilkan "keracunan ginjal".
