Pimpinan Daerah Muhammadiyah Majelis Pembina Kesehatan Kota Surakarta, yang diberi kewenangan mengelola amal usaha Muhammadiyah di bidang kesehatan, merasa terpanggil dan memiliki tekad untuk mengembangkan usahanya, tidak hanya pada pelayanan kesehatan saja tetapi juga pada usaha pendidikan tenaga kesehatan. Maka pada tahun 1993 Pimpinan Daerah Muhammadiyah Majelis Pembina Kesehatan Kota Surakarta mengajukan izin pendirian Pendidikan Diploma III Kesehatan dengan nama Akademi Keperawatan (Akper) Muhammadiyah Surakarta dengan izin operasional nomor HK.00.06.1.1.3180 tanggal 1 September 1993. Pendirian ini sebagai upaya kerjasama dengan pemerintah guna memajukan kesejahteraan umat dan mencerdaskan kehidupan bangsa. Banyaknya Institusi Pendidikan Tenaga Kesehatan yang muncul pada saat itu dan adanya kesamaan nama dengan intitusi lain, maka pada tahun 1994 Pimpinan Daerah MuhammadiyahKota Surakarta menetapkan penggantian nama dari Akper Muhammadiyah Surakarta menjadi Akper PKU Muhammadiyah Surakarta.
Dasar dipilihnya program Diploma III Kesehatan (keperawatan) karena semakin meningkatnya pembangunan di bidang kesehatan, maka dengan sendirinya masalah tenaga kesehatan juga akan mengalami peningkatan sehingga perlu diupayakan dalam hal pendidikan tenaga kesehatan disamping untuk keperluan internal Institusi Kesehatan Muhammadiyah. Harapan didirikannya Institusi Pendidikan Tenaga Kesehatan ini dapat membantu pemerintah dengan menghasilkan tenaga perawat yang profesional dan bertakwa kepada Allah SWT.
Pada tanggal 5 September 2008 Akper PKU Muhammadiyah Surakarta mendapat amanah dari Kementerian Pendidikan Nasional Republik Indonesia untuk mengelola Akademi Kebidanan (Akbid) PKU Muhammadiyah Surakarta dengan Surat Keputusan Menteri Pendidikan Nasional Republik Indonesia No.177/D/O/2008. Selanjutnya pada tanggal 25 Desember 2009 Akper PKU Muhammadiyah Surakarta mendapat izin alih bina dari Departemen Kesehatan ke Departemen Pendidikan Nasional dengan Surat Keputusan No.152/D/O/2009. Pada tanggal 28 September 2011 Akper PKU Muhammadiyah Surakarta mendapat Surat Keputusan Perpanjangan Izin dari Kementerian Pendidikan Nasional Republik Indonesia melalui Koordinasi Perguruan Tinggi Swasta (Kopertis) Wilayah VI Provinsi Jawa Tengah dengan Surat Keputusan No.8813/D/T/K-VI/2011. Kemudian pada tanggal 14 September 2012 Akper PKU Muhammadiyah Surakarta dan Akbid Muhammadiyah Surakarta digabung menjadi Sekolah Tinggi Ilmu Kesehatan (STIKes) PKU Muhammadiyah Surakarta yang diselenggarakan oleh Persyarikatan Muhammadiyah di Provinsi Jawa Tengah melalui Surat Keputusan Menteri Pendidikan dan Kebudayaan Republik Indonesia No.319/E/O/2012.

Pada tanggal 3 Desember 2019 ITS PKU Muhammadiyah Surakarta resmi dinyatakan sebagai institusi perguruan tinggi terakreditasi B oleh Badan Akreditasi Nasional Perguruan Tinggi (BAN-PT) melalui Surat Keputusan nomor 426/SK/BAN-PT/Ak-PNB/PT/XII/2019 yang berlaku sejak tanggal 1 April 2019 sampai dengan 22 Oktober 2022.
